Prescription Refills
Certain medications may be refilled by telephone between 10:00 am and 3:00 pm by calling
770-421-1420. However, if you have not been seen by one of our physicians within the previous four weeks, then an appointment will be necessary to address any prescription needs.

Medications will not be refilled in the following circumstances:
• Medications are lost, stolen, or thrown out
• Missed or cancelled appointments
• Patient has not been seen within the last four weeks

Medical Records Request
You may obtain your medical records by following these simple steps:
1. Click on the form below to open the file and print.
2. Fill out the form.
3. You can mail thecompleted form to us at 335 Roselane Street, Marietta GA 30060, fax the form to 770-421-8055, or bring the form into one of our offices. There may be a fee for reprodction of medical records.

If you have any questions or would like to request your records by phone, please contact our offices directly at 770-421-1420. Record reproductions may be subject to a copy and processing fee, and may take up to two weeks.
