To schedule an appointment at any of the three Non-Surgical Orthopaedics, P.C. locations, please call: 770-421-1420
Our office hours are Monday - Friday from 8:00 am - 5:00 pm. We can often accommodate patients for an appointment within 24 hours. Please bring the following to your Appointment:
- Photo ID
- Insurance Cards
- All Diagnostic Tests - i.e. MRI, X-rays, CT scan
- A list of all current medications
- Medical Records from other Treating Physicians
- Co-payments and / or deductibles
New Patient Forms
To expedite the new patient registration process, you may complete your Medical History and Patient Information Forms in advance of your appointment.
1. Click on the name of each form below to open the file and print.
2. Fill out the forms.
3. Please bring the completed forms to your first appointment or fax them to 770-421-8055 in advance of your appointment. If you choose to fill the forms out at the time of your appointment, please arrive 30 minutes prior to your scheduled appointment time.
