Our office hours are Monday-Friday from 8am to 5 pm. Please call our office at 770-421-1420 to schedule an appointment at one of our multiple locations. Our philosophy is to see all patients within
24 hours. Each visit is scheduled to allow enough time for a thorough evaluation and to answer any questions.

If you must reschedule an appointment, we ask that you call at least 24 hours in advance.

New Patient Forms
To expedite the new patient registration process, you may complete your Medical History and Patient Information Forms in advance of your appointment. If you are interested, please follow the steps below:

1. Click on the name of each form below to open the file and print.
2. Fill out the forms.
3. Please bring the completed forms to your first appointment or fax them to
